Clinic Communiqué

It is always our challenge to effectively communicate our policies to our patients. We thought this would be a good forum. Please let us know if you have questions on any of our current policies.

Children 16 or 17 Years Old - Minors 16 or 17 years old, MUST have a Parent/Legal guardian present for initial office visit or they will be asked to reschedule their appointment. If the patient is 16 or 17 years old, they are able to be seen for their appointment without a Parent/Legal guardian only if Parent/Legal guardian fills out and signs this consent form authorizing Clear Skin Dermatology & Cosmetic Surgery to provide treatment to their teen.

Children 15 Years Old or Younger - Minors 15 years old or younger, MUST have an adult present for all office visits or they will be asked to reschedule their appointment. If the patient is 15 years old or younger, they are able to be seen for their appointment with an adult present other than a Parent/Legal guardian only if Parent/Legal guardian fills out and signs this consent form authorizing Clear Skin Dermatology & Cosmetic Surgery to provide treatment to their child.

Power of Attorney If you have a power of attorney, you must present the office with a copy.

Late Appointments 閖 If a patient arrives 0 10 minutes late for a scheduled appointment, we will fit them in, in the order they arrive. If they are greater than 10 minutes late we will accommodate them into the schedule. If patient is more than 20 minutes late we will kindly ask patient to reschedule.

Missed Appointment Failure to show-up for an appointment could result in a $25 fee.

Cancelled Appointments We kindly ask for a 24-hour cancellation notice.

Insurance A patient without insurance will be required to pay in full upon leaving.

Copays Copays are due at time of service.

Cosmetic Procedures We require payment in full for all cosmetic procedures at time of service.

Patient Due Balances We require payment on the patient due portion of the bill upon checking out.

Copies of Records Please allow 48 hours for processing record copies and there is a small charge.

Disability or Other Insurance Forms Please allow up to 72 hours for processing. There may be a fee depending on the length and time involved to complete the form.
